If you are a new client, please fill out the New Client Form and then return to this page to schedule your appointment.
Step 2: Schedule Your Appointment
You must make an appointment through our online booking system in addition to filling out the form. Thank you!
Returning Clients
Returning clients may book an appointment without filling out the form. If you need to change or cancel your appointment, please use the “view or change your appointment” button at the bottom of your confirmation email.
